Most of our forms are available in several formats. If there is a format you prefer, and we can provide it, please let us know. We are also happy to assist you with questions and will try out best to help you in utilizing our documents.

PDF

(Adobe's "Portable Document Format") has its own "help" page (click here). This includes general information as well as assistance with using the "fillable" (fill-in-the-blank) forms.

Microsoft Word / RTF

Please note that you must have MS Word installed on your computer to use the fill-in forms feature. Don't have Microsoft Word? Click here to download a free MS Word viewer.

The fill-in forms use a feature of Microsoft Word that makes it easy to complete your form or report on your computer. Download the fill-in form document and fill in the form using Microsoft Word. Enter information in the “fields” of the form. To move from field to field, use your mouse, the arrow keys or press Tab. Use your mouse or the spacebar to mark and unmark a checkbox.

If you have difficulty in opening one of our MS Word documents and/or forms, please request that we email you a "RTF" (Rich Text Format) version which should work in any word processing program and is platform independent. Some of our newer forms are already available in this format. Just email MaryAnne for assistance.
