Some documents on this Web site are available as fill-in-the-blanks in MS Word format.
Microsoft® Word documents can be viewed through the use of either Microsoft Word® or the free Microsoft Word Viewer. If your browser is configured properly, documents with the suffix ".doc" will be recognized as Word files and the viewer software will launch automatically. If needed, the Microsoft Word Viewer can be downloaded from Microsoft's web site.
The Microsoft Word Viewer is available for download from Microsoft's web site:
- Click on and download the appropriate form. It is strongly recommended that you first save to your own computer. Note: Some browsers allow you to view MS Word documents within the browser. To save to your own computer's hard disk, right-click on the file icon and select "Save As" or "Save Target As". If you want to keep a copy of the original blank document, use the MS Word “Save As” function and save the document with a unique name and keep the original as a master, for use again in the future.
- Open form and move your cursor to the first field at the top of the page. To enter information on the form, click into each field, or tab from field to field (shown as grey boxes or rectangles; these will not print as grey).
- Save the form to your own computer.
If the form can be emailed (it does not require a signature), please e-mail the document as an attachment back to the Registrar or other appropriate recipient.
If the form requires a signature, print, sign, and send via USPS (or other delivery method) to the appropriate recipient
