* Both fillable-style forms. It is recommended that you SAVE these forms to your own computer (if you want to use the Word form, you MUST save it first; you cannot fill in the information in your web browser). To save to your computer, right-click on the link. Then choose Save Target As or Save As.

When you have completed the information, you can either email the form directly to the registrar, or you can print it and fax or send via traditional mail. If you email or fax, and are going to pay with a check, please print another copy to send with your payment and make a notation that you previously sent a copy of the form (e.g. "COPY - sent original form via email").
